Our office will take your HSA and FSA cards as payment for care.

We also will accept our patients’ out of network insurance benefits in conjunction with their co-pay.

Dr. Cyriacks is not an in-network provider for any insurance company.  Medicare is accepted on a non-par basis.

Payment is due when services are rendered unless prior arrangements have been made.

If you do not have an HSA/FSA, or out of network benefits an itemized receipt will be issued.

Tax receipts are also provided at year end upon request.

   *The office has payment plans, and pre-payment discounts available for those without insurance.

Please note that if you are currently suffering from financial hardship we ask that you let us know.  Care for you will be made available somehow.  We will never let the inability to afford treatment ever stand in the way of healing; you, and the people who love you, are too important.
